数十年前的文献里就记载过乳糖与含有伯胺或仲胺基团的药物发生美拉德反应,反应的过程和产物都很明确了,所以带有伯胺或仲胺基团的药物不适合与乳糖配伍的,至少有发生美拉德反应的可能。
影响美拉德反应的因素主要有乳糖种类(一水乳糖、无水乳糖、喷雾干燥乳糖),水分(湿法制粒)和硬脂酸镁(可加速反应速度)

1. 水分对美拉德反应的影响
1986年,Waltersson考察过primary amine hydrochloride和喷雾干燥乳糖的相容性,250mg primary amine hydrochloride和250mg喷雾干燥乳糖混合后装入中性玻璃安瓿,这些样品分两组,一组直接封口,另一组每个样品分别加入25ul蒸馏水后立即封口,分别在25、70和90°C下放置,于1、2和12周检测含量和外观,结果如附图所示。结果表明,水分加速了主药降解。

2. 相容性实验样品的处理方法
一般的相容性实验样品都是简单混合,这与真正的制剂过程是不同的,制剂过程至少包括制粒和压片,这些单元操作也会影响到原辅料的相容性,因为药物和辅料的分散状态和相容性实验里简单混合的样品的分散状态不同。所以药物和辅料混合后最好压片,再研碎,过80目筛,这样混合的更均匀,药物与辅料接触的更紧密,更接近真实的制剂形态。可以根据片剂里水分含量加适量水分到药物和辅料的混合物里。干法压片的话就不用补充水分了。

3. 伯林格的原研药里面有乳糖,也不能说明其处方是合理的,美拉德反应受多个因素影响,一旦反应意外加速进行,药品质量就不合格了,所以带有伯胺或仲胺基团的药物不适合与乳糖配伍的。

顺提问:硬脂酸镁与铁盐 确实存在配伍禁忌吗? 实际中没有发现啊
